A randomized crossover trial published in Frontiers in Nutrition (NCT05318183) enrolled 39 adults with prediabetes and controlled every meal they ate for two weeks at a time: identical diets differing only in bread — whole wheat in one phase, refined white in the other, separated by a washout. A 75-gram oral glucose tolerance test ran at the end of each phase. Controlled feeding removes the usual self-report noise; the bread was the only variable.

Postprandial glucose excursion fell 27% on the whole wheat phase versus white, independent of body weight, inflammation, and short-chain fatty acids. Fasting glucose and insulin improved over time in both arms. The novelty is in the responders: baseline abundance of specific gut microbes (Lachnospiraceae NK4A136 group and Lachnoclostridium) predicted who got the larger glycemic benefit — an early precision-nutrition signal, not a general excuse to skip the swap.
