Two Diseases Wearing One Name: Lean vs Obese Type 2 Diabetes in 3,300 Adults

A Diabetologia multi-cohort analysis of 3,300+ adults in Amsterdam UMC and University of Ghana datasets found type 2 diabetes runs on two different mechanisms: higher-BMI patients are driven mainly by insulin resistance, lower-BMI patients by failing insulin secretion — with different complication patterns and, possibly, different treatment needs.
The Two Pathways
Analyzing more than 3,300 adults with type 2 diabetes across African and Dutch cohorts, researchers found the disease splits by body composition: in higher-BMI patients, insulin resistance drives the disease; in lower-BMI patients, the pancreas simply under-produces insulin. The two groups also diverge in complications — retinopathy and stroke more common in the lean group, hypertension and cardiovascular disease in the higher-BMI group.
Why One-Size Treatment Fits Poorly
Most patients in both groups receive similar drugs. But therapies aimed at insulin resistance may do less for someone whose problem is insulin secretion — and the study notes that early-life factors like malnutrition or low birth weight can impair pancreatic development, shaping the lean-diabetes pathway decades later. The authors call for mechanism-matched trials before rewriting guidelines.
